I authorize Gen Artistry to apply eyelash extensions, remove eyelash extensions,  perform an eyelash lift and/or tint, and/or perform an eyebrow lamination and/or tint as per my request on this date booked and as well as any future appointments scheduled through the booking website genartistry.as.me.

Eyelash Extensions & Eyelash Extension Removal

I understand and agree to follow aftercare instructions provided by my technician both verbally and direction to my technicians website genartistry.com. I understand failure to follow the aftercare instructions may cause the eyelash extensions to fall out.

I understand in order to have the eyelash extensions applied I will need to keep my eyes closed for a duration of 60-180 minutes. I understand I will be lying in a reclined position. Any medical conditions that might be aggravated by lying still for a prolonged period of time may mean I will not be able to have the procedure performed on my eyes.

I understand there are risks associated with having artificial eyelash extensions applied to and/or removed from my natural lashes including eye irritation, pain, itching, swelling, discomfort and in rare cases eye infection.

I understand and agree if I experience any of these issues with my lashes I will contact my technician and have the eyelash extensions removed immediately and consult a physician at my own expense.

I understand even though the technician may apply and remove the eyelash extensions properly, adhesive material may become dislodged during or after the procedure, which may irritate my eyes or require further follow up care.

Eyebrow Lamination & Eyebrow Tint

I understand and agree to follow the aftercare instructions provided by my technician both verbally and direction to my technicians website genartistry.com. I understand failure to follow the aftercare instructions may cause the natural eyebrow hairs to not laminate and/or tint.

I understand in order to perform the service, I will need to be lying in a reclined position for a duration of 60-180 minutes. Any medical conditions that might be aggravated by lying still for a prolonged period of time may mean I will not be able to have the procedure performed on my eyes.

I understand there are risks associated with having the eyebrow lamination and/or tint procedure performed including skin irritation, pain, itching, swelling, discomfort and in rare cases skin infection.

I understand and agree if I experience any of these issues with my eyebrows I will contact my technician and consult a physician at my own expense.

Eyelash Lift & Eyelash Tint

I understand and agree to follow the aftercare instructions provided by my technician both verbally and direction to my technicians website genartistry.com. I understand failure to follow the aftercare instructions may cause the natural lashes to not lift and/or tint.

I understand in order to perform the service, I will need to keep my eyes closed for a duration of 60-180 minutes. I understand I will need to be lying in a reclined position. Any medical conditions might be aggravated by lying still for a prolonged period of time may mean I will not be able to have the procedure performed on my eyes.

I understand there are risks associated with having the eyelash lift and/or tint procedure performed  including eye irritation, pain, itching, swelling, discomfort and in rare cases eye infection.

I understand and agree if I experience any of these issues with my lashes I will contact my technician and consult a physician at my own expense.

I understand even though the technician may perform the eyelash lift and/or tint correctly, eyelash lift and/or tint material may become dislodged during or after the procedure, which may irritate my eyes or require further follow up care.

I hereby release any and all employees or persons representing this establishment from all demands, claims or damages, actions or cause of actions arising out of the performance of this service.
